Please complete the Baptism Application Form accordingly:
在你遞交洗禮申請表前，請填妥所有資料:
1）Print ALL the information on page 1 & 2, especially the information on page 2 about the ‘Christian Life Series Class”. Remember to ask your teacher to sign for you to affirm that you have completed the course, which is the basic requirement to submit your membership.
以正楷書寫所有列在第一及第二頁的資料。特別留意第二頁有關「信徒生命課程」的部份，緊記請你的教授老師簽署印證你已完成此課程才符合申請的基本資格。
2）Attached with one recent photo. Any picture can show your current outlook will be fine.
附上一張正面相片。任何近照可以清楚看到你的正面均可。
3）Sign the “General Members Covenant” and find your coach/SGL as your witness to co-sign.
請簽署「普通會員信約」並找你的導師/組長在見證人欄上簽署。
4）Submit your “conversion experience” (personal testimony), either handwriting or typing will be fine.
附上你的「信主經歷」(個人得救見證)，手寫或電腦書寫均可。
If you have not completed any of the above mentioned, the office will not be able to proceed your application.
如若上列任何一項尚未完成，教會辦公室將不會接受申請。
